NDIS information

Students and families who already receive a government funded service will be referred to the NDIA (Agency) at Charlestown by that service provider. This has already happened with Newcastle Local Government Area (LGA) residents.

Families who live in the Lake Macquarie LGA will be referred to the NDIA after July 2014 by their Service Provider. These families will be able to participate in the ADHC School Leavers Assessment next term (2), if they wish to. The information gathered will be transferred to the NDIA in September.

Students and families who do not receive any service or have a service provider can self-refer at any time.

There a number of steps to take:

  1. go to the agency office at Hilltop Plaza, Charlestown (the old Rebel Sports space) and talk to reception - OR
  2. make a phone call to 1800 800 110 - OR
  3. complete the My Access Checker on the NDIS website.

If your son or daughter is likely to be eligible, he or she and you will be invited to have a meeting with the agency staff. This is called an Access Request meeting.

It is important that you provide the NDIA staff with as much documented information about your child's disability and support needs as possible, including medical reports, psychology/School Counsellor assessments and school reports.

If you feel you need some support, take a support person to the meeting; someone who knows your child, if possible.
